HOW TO ENROLL
1. Complete one registration form per child. Make a copy for your records.
Write your child’s name in the “memo.”
You may also pay with credit card by filling out the Credit Card Payment Authorization Form.
3. Please submit a separate check payable to GLO. Write your child’s name and indicate Swimming Sessions 1 and/or 2 in the “memo”.
4. Return payment and registration form(s) to 1560 Noriega Street, Suite 206, San Francisco, California
94122 by Friday, May 28, 2010.
OTHER REGISTRATION INFORMATION
10% discount if fees are paid in full by Saturday, April 17, 2010. (GLO Summer Adventures 2010 Registration& Play Day)
$50 per week must be included with the Registration Form.
DCYF Summer Resources Fair Special: $25 Registration Fee for new children will be waived if a GSA Registration Form with a “Fair Special” Stamp is use to enroll child.
A registration confirmation will be mailed to you upon the receipt of your application.
Spaces in extended care hours (7:00-9:00am and 4:00-6:00pm) will be limited.
Enrollment is on a first come first serve basis.
